Completeing a written health history is highly recommended.  There is a lot of information we need to know about you, and by providing this information it saves us time and cuts down the cost of the visit. These are guidelines for writing down your health history.  Please be as complete as possible.  Bring this to your first visit.

This is a standard visit form.  Please complete and bring to your first visit.

Ayurvedic Constitution

This form has basic questions about you that can give us very important information regarding your health constitution.

Medication & Supplement List

This is an easy chart to help us keep track of what you are taking.  Fill it out as thoroughly as possible and bring to your first visit, we recommend that you keep a copy to take to other medical visits.
